Pathways to Work initiative, working in partnership with Jobcentre
Plus, Seetec is delivering the Condition Management Programme in
the Black Country.
Condition Management Programmes are voluntary, bio psycho educational
based provision, delivered by experienced health professionals,
to help new and existing Incapacity Benefit (IB) customers better
understand & manage their symptoms and improve self confidence
leading to them to be better placed to consider other initiatives
to help them return to work
The programme is delivered in partnership
with The Priory and Rehab Works, in order to provide comprehensive
support from physical, social and occupational perspectives.
CMP Practitioners conduct occupational
assessments of IB customers presenting with mild to moderate health
conditions in order to identify what changes an individual might
need to make in their lives, in order to get back into leading the
lives they want to lead.
You will provide condition management
advice through 1:1 case management and/or group interventions and/or
referral to appropriate services.
CMP Practitioners will be required
to possess excellent communication and time management skills and
to use advanced clinical reasoning skills and assessment techniques
to facilitate a return to employment.

therapy, mental health practitioners and similar.
The post may particularly suit someone
looking to develop their careers in health or occupational psychology.
The current vacancy is in Dudley,
but travel may be required across the Black Country region.
